With your permission Miriam will also talk with your family members,
friends and any professionals working with you to get everybody’s ideas on what might help make your life safer and more comfortable in your own home. We may ask you if we can get advice from specialists about, for example, special adaptations that can be made to your home to allow you to continue living there.

We will draw up a care plan suggesting what specialist
equipment you might need and how we might support you. We will also let you know how much this would cost, and we'll work with you to see if there are any ways of getting help funding these costs. If you agree to the plan we have devised we will provide you with the care as agreed.

And then we begin!
We will regularly review the plan with you and make any changes as time goes on. A manager will check in with you and with any friends or relatives that you suggest to make sure that you are well and happy. A supervisor will also check in with you to make sure things are going well and that you're safe in the comfort of your own home and community.
